Sorry I've been out of the discussion for a while, I've been on parental leave. We are not using LastPass on the corporate level and the behaviour has occurred with too many users for there to be any probability that they would be using it individually. I'm not seeing the issue now and I've always had a problem reproducing it (although I have been able to on occassion). Not expecting a solution at this point but I can't call the question anwered!
