It appears that the JQuery custom code fails to locate a HTML element on the page,  please attempt the following:

1. On IE open the developer tool (F12)
2. From the first tab (HTML)  use the arrow and locate the date field that causes the problem and check the name.
3. Use the same name in the custom code and observe and report back upon what results.
