I need a column in a library that stores a hyperlink that says “Send Feedback” to a List.  The link would feed list items from multiple users who are offering feedback on a single document.

The SharePoint Collect Feedback won’t work, since it doesn’t create a link, and you have to start the workflow for it to work to specific users.

I’m thinking I need a qListView Web Part with a one column displaying the Name of the document and another colulmn displaying a default link to a qListForm.  The qListForm would have to automatically populate the Name of the document (and even hide the document name) so that users can randomly send their feedback to a the list.  The list would store a text column for feedback and  a Lookup column to lookup the name of the document.
